トレーニング参加同意書

私は、以下の内容を十分に理解した上で同意し、遵守いたします。

記

  1. コーチの指導のもとレッスンに参加し、自己の責任において安全に十分配慮し、自身の身体的限界を超えない範囲で行動します。
  2. アレルギー体質、負傷中、疾病中、妊娠中、または先天的・後天的な理由により身体機能に障害がある場合は、主治医の承諾を得た上で、参加前に申告します。
  3. 上記2に該当する場合で、緊急時に必要な応急処置がある場合は、必ず事前に申告します。
  4. 上記2に該当する場合は、事前に主治医へ相談し、自己の責任においてレッスン参加に支障がないことを確認し、体調管理を行います。
  5. トレーニング中に体調の異常を感じた場合は、直ちに参加を中断し、コーチへ報告します。
  6. レッスン中に発生した負傷、疾病、後遺症、死亡、流産等については、その原因の如何を問わず、すべて自己の責任とし、関係者に対して一切の責任を問いません。

以上の内容を確認し、同意・遵守いたします。

期間：2026年７月１日 ～ 2027年5月末日

本レッスンに参加するすべての方は、本同意書の内容を確認したものとみなします。

Training Participation Agreement

I hereby acknowledge that I have read, understood, and agree to comply with the following terms and conditions.

Terms

  1. I agree to participate in the lessons under the instruction of the coach, taking full responsibility for my own safety, and to act within my physical limits at all times.
  2. If I have any allergies, injuries, illnesses, am pregnant, or have any physical disabilities (whether congenital or acquired), I agree to obtain approval from my physician and declare such conditions prior to participation.
  3. If I fall under any of the conditions stated in Clause 2 and there are specific emergency procedures that others should be aware of, I will declare them in advance.
  4. If I fall under any of the conditions stated in Clause 2, I confirm that I have consulted with my physician beforehand, that I am able to participate without issue under my own responsibility, and that I will properly manage my health condition.
  5. If I experience any physical abnormality or discomfort during training, I will immediately stop participating and report it to the coach.
  6. I agree that I am fully responsible for any injuries, illnesses, aftereffects, death, or miscarriage that may occur during the lesson, regardless of the cause, and I will not hold any related parties liable.

I confirm that I have read and understood the above, and I agree to comply with all terms stated herein.

Period: July 1, 2026 – End of May 2027

All participants are deemed to have reviewed and agreed to this agreement upon participation.
