    Training Participation Agreement<\/strong><\/p>\n\n\n\n

    I hereby acknowledge that I have read, understood, and agree to comply with the following terms and conditions.<\/p>\n\n\n\n

    Terms<\/strong><\/p>\n\n\n\n

    1. I agree to participate in the lessons under the instruction of the coach, taking full responsibility for my own safety, and to act within my physical limits at all times.<\/li>\n\n\n\n
    2. If I have any allergies, injuries, illnesses, am pregnant, or have any physical disabilities (whether congenital or acquired), I agree to obtain approval from my physician and declare such conditions prior to participation.<\/li>\n\n\n\n
    3. If I fall under any of the conditions stated in Clause 2 and there are specific emergency procedures that others should be aware of, I will declare them in advance.<\/li>\n\n\n\n
    4. If I fall under any of the conditions stated in Clause 2, I confirm that I have consulted with my physician beforehand, that I am able to participate without issue under my own responsibility, and that I will properly manage my health condition.<\/li>\n\n\n\n
    5. If I experience any physical abnormality or discomfort during training, I will immediately stop participating and report it to the coach.<\/li>\n\n\n\n
    6. I agree that I am fully responsible for any injuries, illnesses, aftereffects, death, or miscarriage that may occur during the lesson, regardless of the cause, and I will not hold any related parties liable.<\/li>\n<\/ol>\n\n\n\n

      I confirm that I have read and understood the above, and I agree to comply with all terms stated herein.<\/p>\n\n\n\n
